KLSA LLC & SOCCER WAREHOUSE LLC LIABILITY WAIVER AND RELEASE AGREEMENT

(For Participation in Youth Soccer Training Programs and Facility Activities)

PLEASE READ CAREFULLY BEFORE ACCEPTING. THIS IS A LEGALLY BINDING AGREEMENT.

By clicking “I Agree,” you confirm that you have read, understood, and accepted the terms of this Liability Waiver and Release Agreement. This applies to participation in any activity offered by Kim LiVecchi Soccer Academy (KLSA LLC) and/or Soccer Warehouse LLC, including indoor training programs and facility-based events.

1. ASSUMPTION OF RISK

I understand that participation in soccer training, speed, agility, strength, conditioning, and athletic activities involves inherent risks, including but not limited to: collisions, slips, falls, sprains, fractures, concussions, overuse injuries, muscle strains, joint injuries, and overexertion.

Participation may also carry a risk of exposure to communicable diseases such as COVID-19, even with safety measures in place.

I knowingly and freely assume all such risks, both known and unknown, and accept full responsibility for my (or my child/ward’s) participation.

2. RELEASE AND WAIVER OF LIABILITY
I, on behalf of myself, my child/ward, and our respective heirs, waive, release, and hold harmless KLSA LLC, Soccer Warehouse LLC, and their respective owners, employees, coaches, agents, volunteers, affiliates, and facility lessors from any and all liability for injury, illness, death, or property damage arising out of participation in their activities or use of their facility.

This includes, but is not limited to: private or group training, camps, clinics, games, scrimmages, fitness activities, and facility use.

3. HEALTH AND SAFETY
I affirm that I (or my child) am in good physical condition, capable of safely participating in training and soccer activities.

I understand that KLSA LLC and Soccer Warehouse LLC do not provide medical treatment and that I am responsible for all medical expenses should an injury or emergency occur.

I agree to follow all facility and training program rules, including those related to safety and conduct.

4. PROPERTY DAMAGE & PERSONAL BELONGINGS
I agree that I (or my child) am responsible for any damage to facility property or equipment resulting from negligent or intentional actions.

I acknowledge that the facility is not responsible for lost, stolen, or damaged personal items.

5. COMMUNICABLE DISEASES
I understand that despite reasonable precautions, there remains a risk of exposure to infectious diseases.

I voluntarily assume all such risks and release all associated parties from liability resulting from illness or exposure.

6. MEDIA RELEASE (OPTIONAL)
I grant permission to KLSA LLC and Soccer Warehouse LLC to use photos or videos of me (or my child) taken during participation for promotional use in print, web, or social media.

If I do not wish to consent to this media release, I will notify the organization in writing.

7. ACKNOWLEDGMENT AND AGREEMENT
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND THAT BY ACCEPTING IT I AM GIVING UP SUBSTANTIAL LEGAL RIGHTS FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

I certify that I am the legal parent or guardian of the participant and have explained the terms of this waiver to my child/ward, including the risks involved and the importance of following rules for safety and health. My child/ward understands and accepts these responsibilities. I, along with the other parent/guardian, consent to this release and agree to indemnify and hold harmless the Releasees from any and all claims related to my child’s/ward’s participation in any activities hosted by KLSA LLC or Soccer Warehouse LLC, to the fullest extent permitted by law.
